Well I stole this pic because I dont have a pic of mine, but the exact same thing happened on my 1G AWD transmission.... but mine was much worse.... The dang sleve over the input shaft broke off into 3 pieces and took out the releace bearing which was only about a year old.....

What the heck caused this to happen.... my clutch and flywheel was still good......

Did the tob have a metal inner sleeve or plasic? I would bet it was metal and without looking at your profile, you have a nonstock clutch. With the metal sleeve and heavy clutch, a lot of side loading of the bearing can occur and the extra friction of the metal sleeve will cause it to hang up and a) wear the shaft and/or b) break the shaft.
 
Well if you launch hard alot that can happen or if your clutch is chatterling alot. Either way something has been putting alot of stress on the imput shaft wich has then put that stress on the sleave.
 
Humm, well the old TOB was all melted looking and ripped apart... it may have locked up....

But about the bell housing bolts.... I think you are on to something.... I actually only had 3 holding my engine to my transmission..... now there are 4 there..... when the person before me swapped my car from an auto to a stick I think they forgot one......
 
Thats never a good thing.No wonder why that happened.Are your tranny mounts still good? because if they were allowing that much movement that the tranny moved while missing 1 bolt.. That could be the main source.the missing bolt was an instigator in an already bad situation.
 
My trans mount it fine, all my engine and trans mounts are poly so it is not moving, but I bet that missing bolt had a lot to do with it....
 
didnt you hear any sort of noise from your tranny shifting around?
 
This is what also what happen to me. But my bellhousing is cracked. I'm in the process of installing back my tranny. I only put 3 tranny bolts and 2 starter bolts. I'm going to check for the 4th bolt.
